Care Management Program Services

The Cumberland County Department of Public Health’s high-risk care management programs provide services to the women and children of the County. These services are provided by public health social workers and nurses. The programs continue to work diligently toward achieving positive birth and health outcomes for the women, children and families of Cumberland County by providing excellent quality care.

Services Provided

Care Coordination for Children (CC4C)

Care Coordination for Children is a voluntary program available to all children of Cumberland County from birth to five years with special health care, developmental and social needs. Care Management Services are provided by trained nurses and social workers to assist families in improving health care for their children.

The population of infants and children who have been admitted to neonatal intensive care units (NICU), have chronic medical conditions, live in a toxic and stressful environment or receive foster care services through the Department of Social Services (DSS) has fluctuated over the year. Care Management
focuses on connecting patients with a primary medical home, home health medical services, resolving toxic stress conditions, assist Department of Social Services social workers with continuity of care for children in foster care, developmental therapy and/or therapy and receipt of local and out-of-county medical specialists to ensure they receive necessary care.

Pregnancy Care Management (PCM)

Pregnancy Care Management is a voluntary program for high risk pregnant women in Cumberland County. Services are provided at no cost to the participants. Care managers are highly skilled nurses and social workers trained to assist women in obtaining prenatal care, needed community resources and improved pregnancy outcomes.

Pregnant and postpartum women receive supportive health, medical and educational services to improve and promote healthier pregnancies and births. Pregnancy Medical Homes continue to work with Pregnancy Care Managers by screening and referring patients with high-risk conditions. Care Management encourages early and continuous prenatal care, postpartum follow up care, positive mental and physical health and continuation of primary medical care for health promotion and disease prevention.
